Brooklyn, NY: Deepening its’ community commitment to provide a wide range of quality healthcare services to the vulnerable and underserved communities in Coney Island and South Brooklyn NY, CHI is expanding and launching an affiliate healthcare provider, EveryYou Health.
In creating EveryYou Health, CHI expands the services offered to address the physical and behavioral healthcare needs of the local Brooklyn community and beyond, including all five boroughs of New York City. Services will be delivered both virtually and in person, with specific expertise in a whole-person approach to the proactive management of an active sexual lifestyle, and reduced risk for HIV-1.
With the goal of simplifying prescribing, filling, and refilling prescriptions of PrEP as a proactive care option, EveryYou Health aims to:
- Increase awareness and comprehensive, real-life education about HIV risk and PrEP,
- Simplify the process of qualifying for PrEP and obtaining and refilling prescriptions,
- Provide in-person and remote testing options,
- Improve the sexual and overall health of the community.
Coupled with the launch of EveryYou Health is the introduction of a digital health management program, PrEP Concierge, for patients navigating PrEP treatment. This program is designed in collaboration with GoMo Health, a patient engagement and health management company that leverages BehavioralRx®, its proprietary, evidence-based science of patient motivation and activation to guide successful health management journeys. PrEP Concierge is an interactive, mobile-based program that empowers both those considering PrEP and those already on the PrEP treatment regimen with customized support and information based on each participant’s self-expressed needs.
