1. EveryYou Health Program Overview
Community Health Initiatives, Inc. (“CHI”) is a non-profit Federally Qualified Health Center (“FQHC”) providing health care services. CHI has developed a tele-PreP therapeutic program (the “Developed Program”) designed to make access to PreP services and other health services convenient and accessible. Gold Group Enterprises, Inc. (“GOMO”), d/b/a GoMo Health, provides a suite of scientifically curated health and wellbeing, digital therapeutics, healthcare applications, solutions, and messaging services and provides these and other services via the GOMO Platform. CHI and GOMO have jointly developed a PreP concierge (“Concierge Program”). This website, www.everyyouhealth.com, offers EveryYou Health Concierge Program services free of charge.
We hope that you find the Concierge Program useful in providing information about certain conditions and helpful in making your own health care decisions.
As a participant in the Concierge Program, you can:
- Receive text messages related various medical conditions which are relevant to you.
- Click website links to view educational information and videos related to various medical conditions which may or may not be relevant to you.
- Complete and submit online and/or mobile program health surveys.
The Concierge Program is not designed to and does not provide personalized medical advice, professional diagnosis, opinion, treatment, or services. Medical information provided by CHI is only for general informational and educational purposes. The information sent by text should not be considered as a medical consultation, nor is it meant to replace the provider’s decisions about the appropriateness or risks of a procedure given to a patient. After reading text messages or web pages with general information about certain medical conditions, you should review such information with your medical provider. Never disregard professional medical advice, delay seeking treatment, or rely on general information in lieu of seeking professional medical advice. If you think you may have a medical emergency, call your provider or 911 immediately.
Please also be aware that medical research, professional opinions, and best practices can change over time. 2. Use and Disclosure of Personal Information
CHI respects your privacy. CHI does not sell, trade, or rent your personal information to other companies or individuals. CHI uses industry-leading Secure Sockets Layer (SSL) technology to keep your personal information, when stored on CHI’s cloud-based servers, as secure as possible.
As a visitor to this website or as an EveryYou Health Program participant, you may choose to submit your personal information, including information related to your health or a medical issue, to CHI.
Your personal information may be used for several purposes, including to enable your access to Concierge Program services, to personalize content and to provide additional information about the Concierge Program or another EveryYou Health program.
By registering for the Concierge Program, you give GoMo Health permission to provide your personal information to CHI. CHI and its medical providers (who might be employees or contractors of CHI) will use your personal information to send you personalized text messages related to your primary care encounters, follow-up treatment suggestions or lab work. CHI and its medical providers might also use your personal information, including Protected Health Information as defined by the Health Insurance Portability and Accountability Act (“HIPAA”), for purposes related to medical treatment, payment, and operations. CHI may also provide your personal information (such as your name and emails address) to third-party service providers or affiliates who work on our behalf, for various purposes, including to help us communicate with Program participants, analyze website data, process rewards or payments and provide customer service. CHI also reserves the right to disclose your personal information if it believes in good faith that such disclosure is necessary to (a) comply with relevant laws, (b) respond to legally valid subpoenas or warrants, (c) to protect and defend our rights, property, or (d) to protect the safety or the rights, property, or safety of others.
CHI may also have to report personal information, as well as aggregate information (such as how many medical encounters have been done at the clinic), to law enforcement or other government agencies to comply with applicable laws and regulations,
3. Text and E-mail Communications
As a Program participant, if you send a text message to CHI or respond to a patient satisfaction survey, your mobile phone number and/or their e-mail address, the full content of the message, attached files, or other information provided may be stored by CHI or our service provider’s server logs (See Information Collected Via Technology section).
CHI may use this information to send health-related information or educational content to Program participants. Program participants may request to stop receiving communications or opt-out at any time. (See Your Choices Regarding Your Personal Information section).
Since text messaging is not a secure way to transmit or share personal information, including health-related information, you agree not to send personal information, including but not limited to information about your health or a medical issue, to CHI via text message.
4. Information Collected Via Technology
CHI may also collect and analyze non-personal information concerning visitors to everyyouhealth.com web page, using de-identification processes to help ensure that such information cannot reasonably be re-associated or connected with participants. Thus, if you use this website and participate in the Program, certain information may be passively collected and stored on CHI’s and our service providers’ server logs, including your device Internet protocol (“IP”) address, browser type and operating system. We also use Cookies and website-related data like Uniform Resource Locators (“URLs”) to gather information such as the date(s) and time(s) of your web page visit(s) and the information viewed. Generally, this information is used to analyze website performance, potential website enhancements and patterns of use. “Cookies” are small pieces of information that a website sends to a device’s hard drive while during a website visit. The eveyyouhealth.com website may use both session Cookies (which expire once the web browser closes) and persistent Cookies (which remain on the computer until deleted) to provide a more personal and interactive experience. Persistent Cookies can be removed directly in Internet browser settings. Likewise, Program participants may manage Cookie preferences via their browser settings (see www.aboutcookies.org for more information).
An IP address is a number that is automatically assigned to a participant computer when it visits the Internet. In some cases, an IP address may stay the same from browser session to browser session. If you use an Internet service provider (ISP), your device’s IP address might vary from session to session.
CHI also uses Google Analytics to obtain non-personally identifiable information, including but not limited to data related to where devices came from before visiting www.everyyouhealth.com, what actions were taken while visiting this website and other websites visited after this website.
This Privacy Statement does not limit use or disclosure of information that does not itself identify an individual user. CHI reserves the right to use and disclose such information to Program partners, service providers, sponsors and other third parties at our discretion.
5. Changes to CHI’s Corporate Structure or Ownership
In the event CHI goes through a business transition such as a merger, acquisition by another entity or sale of all or a portion of our assets, your personal information may be among the assets transferred. You acknowledge and agree that such transfers may occur.
6. Your Choices Regarding Your Personal Information
As a Concierge Program participant, you have the right at any time to opt out of the program. To opt out of receiving text messages, reply “STOP” at any time after receiving a text message. If you provided an email address for communications related to the Concierge Program, you may “opt-out” by following the unsubscribe instructions provided in any e-mail from.

7. Updates
This Privacy Statement might be updated periodically and without prior notice to you to reflect changes in our information practices or relevant laws. We will post a notice on www.everyouhealth.com, and any other CHI websites that point to this Privacy Statement, to notify you of any substantive changes to the way we collect and use personal information. We will also indicate at the top of this Privacy Statement when it was last updated.
